U.S. 178 travels 233.23 miles southeast across the Palmetto State from the North Carolina state line in the Blue Ridge Mountains to U.S. 78 near Dorchester in the LowCountry. Through the Upstate, U.S. 178 joins the Pickens County seat of Pickens with Liberty and the Anderson County seat of Anderson. U.S. 76/178 overlap 23.54 miles east from Anderson to Honea Path.

Continuing southeast, U.S. 178 heads to Donalds in Abbeville County and Hodges in Greenwood County. U.S. 176 combines with U.S. 25 south 15.77 miles through the city of Greenwood before turning east to Saluda. Passing through western Lexington County, U.S. 178 connects Batesburg-Leesville with I-20 and Pelion.

U.S. 178 meets U.S. 321 at North in Orangeburg County and then proceeds southeast parallel to the Edisto River North Fork to Orangeburg. U.S. 178 partitions with U.S. 178 Business, a 4.40 mile long route through Downtown Orangeburg while bypassing the city along overlaps with U.S. 21 and U.S. 601 Truck. U.S. 178 follows the I-26 corridor southeast from the remaining miles to Bowman, Harleyville and U.S. 78 in Dorchester County.
