U.S. 15 begins at the intersection where U.S. 17 Alternate turns from Jefferies Boulevard onto Wichman Street on the outskirts of Downtown Walterboro. Jefferies Boulevard extends north along U.S. 15 across Ireland Boulevard, doubling as S.C. 64 Business for 0.40 miles. Becoming a rural highway, U.S. 15 parallels I-95 to the east from Walterboro to S.C. 61 at Canadys and U.S. 78 in the town of St. George.

Continuing northeast, U.S. 15 crosses paths with U.S. 178 at Rosinville, I-26 at a cloverleaf interchange near Four Hole Swamp and U.S. 176/S.C. 45 at a five point intersection at Wells. U.S. 15 arcs westward, meeting I-95 at Exit 93 and again at Exit 97 alongside U.S. 301. U.S. 15/301 combine with Interstate 95 northeast 11.14 miles to span Lake Marion from Santee into Clarendon County.

The original two lane span taking U.S. 15/301 across Lake Marion was declared unsafe for vehicle travel in 1987. The closure resulted in the realignment of U.S. 15/301 onto I-95, which was approved by the American Association of State Highway Officials (AASHTO) on December 5, 1987. The former span remains in place along the north side of Interstate 95.

U.S. 15/301 branch northward from I-95 at Exit 102 and parallel the freeway to their separation in Summerton. U.S. 15 continues from Paxville to the city of Sumter, where it meets U.S. 521, U.S. 76 Business and U.S. 401. Turning more to the east, U.S. 15 enters Lee County ahead of interstate 20 and Bishopville.

Within Darlington County, a 6.50 mile long business route for U.S. 15 loops through Hartsville while U.S. 15 bypasses the city to the east. U.S. 15 Business was designated at Hartsville with approval by the American Association of State Highway Officials (AASHO) on July 5, 1966. The same action established the new alignment as U.S. 15 Bypass. U.S. 15 Bypass was eventually renumbered as U.S. 15 at the request of the South Carolina Department of Highways and Public Transportation and with concurrence by AASHTO on June 26, 1979.

U.S. 15 combines with U.S. 52/401 for 2.08 miles leading onto the town of Society Hill. U.S. 52 branches northwest to Cheraw while U.S. 15/401 remain paired northeast to Bennettsville, Tatum and McColl.
