Forming part of a multi state route north to Rock Hill, South Carolina, Florida State Road 121 extends 102.8 miles south from the St. Marys River and Charlton County, Georgia to U.S. 19/98 and Levy County Road 336 at Lebanon Station. SR 121 passes through Goethe State Forest northeast from southern Levy County to the city of Williston. SR 121 dog legs through Williston along concurrences with U.S. 41 and U.S. 27 on SW 27th Street, West Noble Avenue and Main Street. NE 6th Boulevard leads the route out of Williston, with SR 121 crossing the Alachua County line west of Barr Hammock Preserve.

Through Gainesville, SR 121 follows Williston Road to Interstate 75, and SW 34th Street north from SR 331 to the University of Florida (UF) campus and SR 26 (University Avenue). NW 34th Street leads the state road through neighborhoods of north Gainesville to U.S. 441. Departing Gainesville, SR 121 resumes a rural course to La Crosse, Worthington Springs and the Union County seat of Lake Butler.

The northern extent of SR 121 connects SR 100 and Lake Butler with the town of Raiford at SR 16 and the Baker County seat of Macclenny. Following 6th Street through Macclenny, SR 121 meets I-10 at a parclo B2 interchange and U.S. 90 in Downtown. Georgia State Route 121 combines with SR 23 north from the St. Marys River to Folkston.
